This statement covers praxisally.com, its forms and the tools on it. Client engagements are governed by the written scope.

What you choose to share

The contact form asks for your name, organization, email address, the kind of support you need and a description of your priorities. Optional fields ask for your country or region, the kind of engagement, timing and how you found Praxis. The RFP form asks for a contact name, agency or organization, email, optional phone number, agency type, submission method, deadlines, the public solicitation link and an optional scope note. Forms also include the form language, selected sector context, a confirmation destination and an empty spam-prevention field.

Where your message goes

Formspree processes the form and forwards inquiries to hisham@praxisally.com. Praxis uses your message to respond, discuss a potential engagement and maintain the related correspondence. Formspree, the website host and the email provider also process technical information under their own service and privacy terms.

Praxis Readiness Review and first-month planner

The Praxis Readiness Review calculates scores in your browser from your answers. Your answers and results are not transmitted until you choose to share them with Praxis. The score is a self-reported planning diagnostic, not a compliance certification or prediction of funding. You can print or save the results without sharing them. The form includes organization type, budget range, funding mix, timing and answers about needs, outcomes, budgets, ownership, records, reporting, controls and partners. If you share the review, it includes the assessment version, results, organization, email address and your consent; a subject line routes the message.

The first-month planner creates an illustrative scope on your device. Moving that scope to Contact uses temporary session storage in your browser; the stored scope is removed when imported. It does not send an inquiry automatically. Submitting the contact or Readiness Review form sends the selected information through Formspree to Praxis. Praxis does not promise an automatic results email.

With JavaScript off, the Readiness Review form sends your answers and email to Praxis for a manual review when you submit it. The planner displays a static sample scope. Neither tool collects patient records, account credentials or confidential case information.

Retention and requests

Inquiry messages may remain in the form service and email correspondence while they are relevant to the conversation or related business records. Praxis does not set a fixed automatic deletion period for inquiry messages. To request access, correction or deletion of an inquiry you sent, email hisham@praxisally.com. Praxis will review your request and any records that need to be retained.

Website preferences and measurement

The motion preference is stored in browser localStorage under praxis-motion. Referral labels are included only with an inquiry you choose to submit. Neither the motion preference nor referral labels set cookies or record browsing history. Praxis does not add advertising or analytics cookies. ChatGPT Sites, the website host, and Cloudflare, its network provider, set the cookies listed below. Praxis does not set cookies of its own.

Language choices are links to separate pages; this site does not store a language preference.

The hosting providers also set these cookies on file responses, including downloads and images. Cloudflare uses __cf_bm to identify and mitigate automated traffic.

Page performance

The website measures page loading, visual stability and interaction speed using a self-hosted script. It sends only a public page path, the metric name and its value to this website. The performance script itself creates no visitor identifier or cookie. Values are combined into weekly performance ranges and kept for 13 weeks; raw individual measurements are not stored. The hosting provider may process technical request information under its own policies.

International transfers

Praxis is based in the United States. The hosting provider delivers this website, Formspree processes submitted inquiries and Google Workspace handles business email. These providers may process information in the United States and other locations under their service arrangements. Contact Praxis before sharing information that must remain in a particular jurisdiction. Praxis does not promise United States-only storage without verified contractual controls.

Access, correction and consent

You may ask to access or correct information you supplied, or ask Praxis to stop processing that relies on your consent. Withdrawal does not undo earlier processing and may affect Praxis’ ability to respond or provide the requested service. Praxis will explain any records that must be retained for applicable obligations.
