What you choose to share

The contact form asks for your organization, email address, the kind of support you need, and a description of your priorities. Optional fields ask about the kind of engagement, timing and how you found Praxis. A referral link may preselect a source, which you can change before submitting. Please do not include patient records, passwords, financial account details, or other confidential information.

Where your message goes

Formspree processes the form and forwards inquiries to hisham@praxisally.com. We use your message to respond, discuss a potential engagement, and maintain the related correspondence. Formspree, the website host, and the email provider also process technical information under their own service and privacy terms.

Readiness assessment and first-month planner

The readiness assessment calculates scores in your browser from your answers. Your answers and results are not transmitted until you choose to share them with Praxis. The score is a self-reported planning diagnostic, not a compliance certification or prediction of funding. You can print or save the results without sharing them.

The first-month planner creates an illustrative scope on your device. Moving that scope to Contact uses temporary session storage in your browser; the stored scope is removed when imported. It does not send an inquiry automatically. Submitting the contact or assessment form sends the selected information through Formspree to Praxis. An automatic results email to the respondent is not promised.

With JavaScript disabled, the assessment form sends your answers and email to Praxis for a manual review when you submit it. The planner displays a static sample scope. Neither tool collects patient records, account credentials, or confidential case information.

Retention and requests

Inquiry messages may remain in the form service and email correspondence while they are relevant to the conversation or related business records. A fixed automatic deletion period is not currently configured. To request access, correction, or deletion of an inquiry you sent, email hisham@praxisally.com. We will review your request and any records that need to be retained.

Website preferences and measurement

The motion preference is stored in your browser so the website can respect your choice. Referral labels are included only with an inquiry you choose to submit. They do not set cookies or record browsing history. No advertising tracker is included in this website. Cloudflare Web Analytics is not enabled in this release.
